چکیده

The retrograde approach for percutaneous coronary intervention (PCI) of chronic total occlusion (CTO) has helped to improve the success rate. It is usually performed through collateral connections that originate from the contralateral coronary artery to the occluded one and by using two arterial accesses and two guiding catheters, most commonly from the femoral approach. A first reported case of epsilateral retrograde PCI to CTO of the left anterior descending artery (LAD) via the radial approach, using a single guiding catheter and through epsilateral collaterals originating from the LAD itself, is presented.

Retrograde Percutaneous Coronary Intervention for Left Anterior Descending Chronic Total Occlusion Via an Ipsilateral Intraseptal Collateral Channel Using a Single Guiding Catheter.

Successful recanalization of chronic total occlusion (CTO) of coronary arteries has improved symptoms and mortality. In CTO cases, retrograde approach from opposite coronary artery has a better chance of procedural success. But the retrograde approach from opposite site is not always suitable. In certain CTO cases, the distal left anterior descending (LAD) artery is supplied from the intrasepta...

Recanalization of a Coronary Chronic Total Occlusion by a Retrograde Approach Using Ipsilateral Double Guiding Catheters

The retrograde approach through a collateral artery is now thought to improve the success rate of percutaneous coronary intervention (PCI) for coronary chronic total occlusion (CTO), and different kinds of strategies for this technique have been developed. However, the basic principles of PCI for CTO, such as firm back-up support with a guiding catheter and fine control of the guide wire, shoul...

Angioplasty of an occluded left anterior descending coronary artery: usefulness of retrograde opacification of the distal part of the occluded vessel via the contralateral coronary artery in positioning the balloon catheter.

A 59 year old man was admitted to hospital with a non-transmural anterior myocardial infarction. Recurrent angina pectoris eight days after the initial infarction was investigated by cardiac catheterisation, which showed moderate anterior hypokinesis and proximal occlusion of the left anterior descending coronary artery. The distal part of this vessel was opacified via collaterals from the righ...

Balloon-assisted tracking for challenging transradial percutaneous coronary intervention

A-74-year-old woman with a history of diabetes mellitus and hypertension was admitted to our hospital due to non-ST elevation myocardial infarction. Coronary angiography was performed uneventfully using JL 3.5 and AR1 5 Fr angiographic catheters (Boston Scientific, USA) through radial access. The coronary angiogram showed one vessel disease with an 80% stenosis at the proximal part of the left ...

Bilateral Trans-Radial Percutaneous Coronary Intervention with Retrograde Approach for Chronic Totally Occluded Left Anterior Descending Artery

Despite advances in new devices, equipment, and expertise, percutaneous coronary intervention (PCI) for chronic total occlusion (CTO) remains difficult and technically challenging. In recent years, development of the retrograde approach via intra-myocardial septal or epicardial collaterals has improved its success rate, even in cases of antegrade PCI failure. Most PCI experience for CTO was usi...
